To save time, you may call your pharmacy before your visit and request them to fax our office your current list of medications at (780) 533 6374.

Please AVOID BRINGING YOUR PRESCRIPTION BOTTLES. Due to Covid-19, our staff will no longer be allowed to handle the bottles.

 

Your prescription eyeglasses.

 

Medical History

This includes information on your present health issues, your complete surgical history, and your family's medical history, as well as a record of all allergies and reactions.

 

Surgical History

If you've undergone previous LASIK/PRK or any other eye-related surgeries, kindly bring along any pre-operative information you may have.

*Attention Contact Lens Wearers: Please refrain from wearing soft contact lenses for at least one week before your appointment and hard contact lenses for two weeks before your appointment.*
